Ready to become a patient at Lovelace Family Medicine? Getting started is simple. To begin the new patient process, please complete the two steps below. First, complete and submit the New Patient Intake Form so we can learn more about you and your healthcare needs. Next, complete and submit the Release of Medical Records Form, which allows us to obtain your medical records from your previous healthcare provider.

 

Once both forms have been received, our team will review your information and contact you regarding the next steps.
