Current patients are welcome to call (803)364-4852 at any time to set up an appointment.
Lovelace Family Medicine is proud to offer same day appointments with each of our providers in order to accommodate our patient’s busy schedules and to still allow them to see the provider of their choice.
As you know, you may be asked to fill out some paperwork upon arrival to our office. In order to make your appointment go as smoothly as possible we have included those forms in the links below. If you are able to, we ask that you please print these forms out and fill them out ahead of time to hand to our receptionist upon your arrival at the office.

Release of Medical Records
In order to keep your medical information confidential, medical records will only be released once our office has received a signed authorization form. You can stop by the office and request a form to fill out or you may print the form at the link below and mail, fax, or drop the form off at the front desk.​
New Patients
​If you are interested in becoming a new patient, we will need is a copy of your previous medical records. There are two easy ways to do this.

Ask your previous doctor’s office to send us a copy of your records.

Print a copy of our medical records request form, and mail, fax, or drop this form off at our office and we will request a copy of your records from your previous doctor for you.

Once we have received a copy of your records, a provider of your choice at Lovelace Family Medicine will personally review them. After your records have been reviewed, your provider will notify the front desk to contact you to schedule your first appointment with the knowledge that your provider is already familiar with you, your particular medical needs, and what type of appointment is best for you.​
​When you arrive for your first appointment you will be asked to fill out our new patient paper work. If you would prefer to fill this out ahead of time you are more than welcome to print the forms from the following links and bring them with you to our office on the day of your visit.

Fees and Insurance Policy
We will be happy to explain our fee schedule at any time. Please ask to speak to someone in our business office if you have any questions about fees. Payment is expected at the time of service unless prior arrangements have been made. For extensive services, we can arrange a monthly payment plan or other option if necessary. We contract services and will file your insurance for the following insurance carriers:

Please note that we do not accept any Medicare Advantage plans.
Please check with our front desk for any additions or deletions to this list. You are always responsible for payment of non-covered services, co-payments, or deductibles at the time service is rendered.
Referrals
In the event your provider feels it is necessary refer you to a specialist we ask that you give our office 7-10 business days to set up your appointment. If you have not heard from us or the specialist’s office after that time please call our office to check on your referral. Tricare and Bluechoice patients, please be aware that if you plan to continue to see a specialist you may need to extend your referral beyond the original time-frame. If this is the case please call our office in advance of your next appointment date so that we will have time to request an extension of your authorization.

Patient Portal
The Patient Portal allows for secure communication and transfer of information between Lovelace Family Medicine and our patients while ensuring your privacy is in compliance with federal and state guidelines. We do not accept health communications via e-mails. If you have a need or concern requiring immediate attention from your provider, please call the office directly at 803-364-4852.
